When this is the case there’s often still the question if the bacteria is the cause or the result of the prostate complaints. In some cases nothing is found in the culture. However, the pain remain.
The prostate and prostate hypertrophy, and the prostatism complaints
In half of all men over 50 the prostate is swollen, which is called is called prostate hypertrophy. This isn’t necessarily a problem, but could be the cause of prostate complaints like difficulty urinating etc. That entire range of complaints is called prostatism. The doctor can check if there is nothing malignant hiding behind the swollen prostate. The first step in finding out is the so-called rectal examination. In this case a finger is used to inspect inside of the rectum and feel for the size and consistency of the prostate. An enlarged prostate with lumps will mean that the urologist will continue to examine if prostate cancer is present.
If it turns out that there are suspicious lumps in the enlarged prostate it doesn’t have to be the end of the world. It is very possible that it’s a case of benign prostate cancer that has been there for years. Nowadays, removing the prostate in total mostly is an unnecessary procedure. Fortunately because the prostate is surrounded by small nerves that could be damaged when the prostate is removed. A result of this is the inability to erect the penis, so causing impotence.
Prostate complaints, chronic pain and no bacteria
You suffer from chronic pain caused by the prostate, but no bacteria have been found in the culture. Or maybe bacteria have been found and you get antibiotics. A war is being initiated without a cause, because there are no bacteria.
The prostate and the general, the mast cell
Late Nobel Prize winner professor Levi-Montalcini, who lived up to be 103 years old, used to call one of the most important cells in this inflammatory process the prima donna cell of inflammation. This mast cell.   This cell is the general in the war without an enemy and it recruits, an army of inflammatory cells and inflammation mediators. This causes a flood of problems, including pain in the pelvis and problems urinating.
Professor Montalcini studied the mast cell and inflammation mediators her entire life and was awarded the Nobel Prize for her work in 1986. This didn’t stop her from continuing her work. On the contrary, she kept on working hard and at almost 90 years of age she discovered a very special anti-inflammatory and pain reliever that calms the mast cells, called palmitoylethanolamide.
This substance is a body’s own fatty acid and what’s special about it is that it both has a pain relieving as well as an anti-inflammatory effect. There’s no need to worry about nasty side effects because this is a natural substance produced by our own body. What is this substance called? Prostatodynia, painful pelvic floor and interstitial cystitis
Here we will share more details on prostate pain. There is a difficult medical term for prostate pain, namely prostatodynia.
The pain can be quite aggravating, burning, sharp, nagging or even a biting pain. Problems urinating are not always continuously present as these usually come in waves. There will be periods without complaints. The pain can increase during cold or wet weather, psychological stress, nervousness, alcohol use and even heavily spiced food and sitting for long periods.
Prostate pain is not a medically dangerous condition. It is not the pre-stage of prostate cancer, as some people think it is. It’s not harmful but it is inconvenient and can negatively affect the quality of life and sexual relationships. Painful prostate for men and painful pelvic floor for women. It’s probably useful to discuss the female variation on prostate pain. It is a similar inflammation, but located on the wall of the bladder. The difficult word for this is interstitial cystitis. The cause for interstitial cystitis is not knows, same as in the case of prostate inflammation. It is also an escalated inflammatory response, located on the wall of the bladder, caused by different possible factors and which could be aggravated by stress.
Interstitial cystitis (IC)
Interstitial cystitis has been described in medical literature as early as the 19th century. It was predominantly the name Hunner that was connected to this. Hunner, a surgeon from Boston, described the condition with a series of patients in 1925. He described the condition with women aged 20-40 that showed the typical symptoms as mentioned above. Added to those was dyspareunia, which means pain during intercourse. Hunner discovered that these patients had an extremely sensitive bladder mucus layer.
The cystoscopy (possible even then!) didn’t show any abnormalities at first, but after contact with a piece of cotton the mucus layer immediately started bleeding, similar to an ulcer. Later this was named the ulcer of Hunner. Hunner believed a deeper inflammation of the bladder was concerned, therefore the name interstitial cystitis. He treated his patients with silver nitrate bladder irrigation, which mostly had a temporary effect.

On top of assessing the mucus layer, the capacity of the bladder is also inspected, meaning the amount of urine the bladder can hold. It is also important to remove some tissue form inside of the bladder (a biopsy) and have these biopsies inspected microscopically. These biopsies often show an increase in mast cells that entered the bladder covering. 
When in contact with normal tissue, mast cells produce histamine that causes pain, swelling, increased blood supply and later on scar tissue in the affected organ. (End of quote)
